SMTP server error: 5.5.1 Error: nested MAIL command

I'm trying to send campaign email for sugarcrm around 800 of them.  When it hit number 520 something it stop sending with this kind of error.

Did some googling and some people mentioned about amavisd bug but that was in 2008.  Wouldn't it be fair to assume that by 2012 those bugs are already patched?  Am I looking at the wrong problem?  Please advise?  I'm very puzzled.  Many thanks in advance for your help Mr. Zhang.
